I am interested in my child receiving services at THRIVE. Now what?
Please contact us at 850-518-7611 to schedule an evaluation appointment with one of our experienced therapists. If you are unsure if your child would benefit from working with us, please consider signing up for one of our no cost screening events.
At this time, we are working with self-pay, certain Medicaid plans, and SUFS FES-UA Scholarship (formerly Gardiner) clients.
We will provide completed claim forms needed for filing with your insurance while our enrollment process is pending for out of network insurance. Please note that most insurance plans include an "out of network" benefit. Do not hesitate to ask for our help in determining your benefit.
